Long time! I think most of the time, it is state of the relationship and then the heart!
I’d say relationship, since it necessarily involves the other person’s state as well.

One would hope(?) twas the heart, but alas it always is the relationship.

re: your question: i would agree with the other commenters-first the relationship then the heart. healing a broken heart has always been a long and somewhat tedious road for me!
